What Is Chronic Care Management?
Chronic care management (CCM) is a specific care management service that provides coverage for individuals diagnosed with two or more chronic diseases, facilitating an ongoing relationship with their individual care team. This includes developing a comprehensive care plan, coordination of care between providers, medication management, and interactive remote communication and management (generally over the phone).
Chronic care management is available at Digestive Health Center of MS and is often helpful for patients when it comes to receiving continuous health and wellness support. It also enables increased access to relevant healthcare resources, enhanced communication with care team members, fewer ER visits, hospital stays, or readmissions, and increased participation in their own healthcare.
